Friend and follower of Mukuro, Ken broke out of prison with him. He has a scar that stretches across his face. Like Mukuro and Chikusa, Ken was also experimented on by the Estraneo Famiglia. The experimentations done on Ken involved giving him attributes of certain animals when he changes to different sets of teeth. He decided to follow Mukuro when the boy killed their captors, and offered them to come with him. He was Chikusa's cellmate in jail and often calls him "Kakipi". When Ken changes to a different set of teeth, he could adopt the abilities of various animals, like wolves, lions, cheetahs, gorillas and even rhinoceros. He dubs his various forms "channels", comparing it to changing game cartridges. With them, his agility, strength, and senses increase. He was defeated by Yamamoto and was later defeated once again by an injured Kyoya Hibari. Despite his injured body being used by Mukuro during the latter's battle against Tsuna, he remains extremely loyal to Mukuro. He, along with the rest of the gang, was taken away by the Vendicare, but later escaped. Ken often mocks and berates Chrome every chance he gets, showing his evident dislike for the girl.

He seems to love taking showers and staying clean and often tells Ken to take one himself. But, that belies his real talent as a professional assassin, using his deadly "Hedgehog" yo-yo's to spray his opponents with poisoned needles. He talks only if necessary; though, he is often shown to argue with Ken. Like Ken, he also cares for Chrome, but never shows this openly and sometimes "teases" Ken, who always denies his feelings of concern for Chrome. He has been Mukuro's subordinate since childhood along with Ken and is extremely loyal to him. He will follow any of Mukuro's orders and is willing to sacrifice his life for him.

Portrayed as the series' first primary antagonist, Mukuro Rokudo is introduced as a 15-year-old Mafia criminal who is the leader of the Kokuyo Gang, which consists of heinous criminals whom have recently escaped from prison. However, later on in the series, he takes on a more supportive role for the series' main characters, becoming somewhat of an ally rather than an enemy, though he prefers to not consider himself as one of their allies. Despite often being shown with a playful smile, Mukuro is seemingly apathetic to the suffering of others. Appearing as the series' first antagonist, Mukuro is not averse to disposing of those who get in his way, and is not easily intimidated, usually speaking to others in a very direct and arrogant manner. He cares little for others, and simply considers people to be "toys" or "tools" he can sacrifice in order to get to what he wants. Mukuro is implied to have a strong dislike for the entire Mafia underground, due in part to the experimentations he suffered as a child at the hands of the Estraneo Mafia family. When Mukuro killed his captors, he offered Chikusa and Ken, who were also victims of experimentation, a chance to join him in his quest to destroy the world. However, not only does he want to take revenge on the Mafia by destroying it, he also wants to cleanse the world of its filth by invoking universal suffering through a world war. Even after becoming the Vongola Mist Guardian, Mukuro still claims to no longer be part of the Mafia, having been exiled from it, and continues to view the organization with great contempt.

Dino is the 22-year-old 10th generation boss of the Cavallone crime family, and was born on February 4 in Italy, with a blood type of O. Dino is a young man with great skills as a fighter and as a leader. His main weapons are a whip and his pet turtle, Enzio. Like Tsuna, Dino, at first, did not want anything to do with the Mafia and was also a klutz. He became a suitable Mafia boss after Reborn became his home tutor. He came to be known as "Bucking-Horse Dino" and fixed the Cavallone's financial problems, turning them into the third most influential family. Unfortunately, Dino is so devoted to his family, that when none of his family members are around, his skills decrease dramatically, often accidentally hitting people with his whip and falling down the stairs. Dino has a large tattoo sleeve that covers his left hand and most of his arm. According to Futa, Dino is the Mafia member who cares the most for the welfare of civilians.

He is most notable for his long silver hair, and is always seen wearing his Varia uniform in both the anime and the manga. He is a very loud character, and has a tendency to shout "VOOOI!!!" (the Italian version of the plural "You") before saying something or while in battle. Squalo is known as one of the "Vongola's Sword Masters", other being Yamamoto Takeshi. For that reason, Yamamoto somewhat admires him as a awesome swordsman. He was also Yamamoto's tutor. He is supposed to represent the sin of Pride since his name, Superbi, comes from "superbia", Latin term for pride.
